Fax Form 4506-T to ask for a tax transcript. Individual transcripts (Form 1040, W-2, and Form 1099) and other transcripts go to different RAIVS teams.
The fax number on the IRS where-to-file page can differ from the number printed on the form. Use the number on that page.
What to include
Pick the row for the transcript you want and the state you lived in, or where the business was, when that return was filed. If you need more than one transcript and the charts name two offices, use the office for your most recent return.
